- NEET (UG)-2025/NATA-2025 Score Card, if applicable
- Migration Certificate
- Receipt of fee remitted by Online/through Head Post Office in Kerala
- Eligibility certificate from the University concerned, in the case of candidates who have passed a qualifying examination other than HSE/VHSE conducted by Government of Kerala of the examination conducted by CBSE and CICSE.
- Physical Fitness Certificate in the relevant format given in Annexure XVII of the Prospectus
- Originals of all the documents/certificates upload during the online application.
- Any other documents required in the allotment as per notification.
