- Originals of all the documents/certificates uploaded (to prove nativity/reservatio/fee concession) to the online application.
- Data sheet of PG Dental 2022 issued by CEE.
- Allotment memo issued by the commissioner for Entrance Examinations.
- SSLC or any relevant record to prove date of birth.
- Pass/Degree certificate of the qualifying examination (BDS Degree).
- Registration Certificate from Kerala Dental Council & Up-to-date Renewal certificate.
- CRRI Certificate.
- Certificate of Eligibility/Equivalency certificate from KUHS for candidates who have passed the qualifying examination (BDS Degree) from Universities outside kerala.
- NEET (MDS) 2022 Score Sheet.
- Transfer Certificate and conduct certificate from the institution where the candidate studied last.
- Relieving order from the institution last worked for those in Government service.
